Examinations/Required Forms is a Graduate College website listing examinations and forms required of all graduate degree seeking students.
Recommendation for Committee Appointment should be completed as early as possible but no later than the semester before the preliminary oral examination (for Ph.D. students) or the final oral examination (for all other students).
Request to Change Committee Appointment form is used only if you have a Graduate College approved Committee Appointment on file and you need to change a committee member.
Program of Study (POS) should be completed as early as possible after the committee has been approved by the Graduate College, but no later than the semester before the preliminary oral examination (for Ph.D.) or the final oral examination (for all other students).
Program of Study Supplement or page 2 is used when you want to list more courses than will fit on the Program of Study form.
Modifications to the Program of Study form is used if you were unable to take a course listed on your approved Program of Study.
Application for Graduation (Diploma Slip) is submitted at the end of the first week of classes of the semester of graduation (Fall and Spring) or by the last day of Spring semester for Summer graduation.
Department forms were developed to be used before Graduate College degree program forms are filed, or for record keeping proposes.
A Request to Change Degree Programs is used in very unusual circumstances. Examples include changing from the Ph.D. degree to the M.S. degree or changing of majors from electrical to computer engineering.
The Request to Declare or Change Adviser/Major Professor could be filed to declare a co-major professor or to change from one adviser to another within the department. Both forms require signatures from your admitting adviser and your new major professor and then submitted to Graduate Student Services for approval.
Provisionally admitted students complete the Request to Change From Provisional to Full Admission form when background course deficiencies have been met. NOTE: If admitted provisionally to take first year graduate level coursework, this form is NOT required and Graduate Student Services will automatically change your status at the end of the first year of study.
When registering for the Electrical Engineering or Computer Engineering Special Topics course, the form EE/CprE 590 Special Topics Form needs to be completed and submitted to Graduate Student Services. After receiving the form, the Graduate Secretary will give you the correct registration number. If the course is to be included on the Program of Study, a final written report will be need to be submitted to Graduate Student Services.
In cases of emergencies, we need to be able to contact relatives or friends that can help you. Please fill out the Emergency Sheet and return it to Graduate Student Services. Please keep us informed of changes in the information throughout your time with us.
In most cases, you will be assigned keys for the outside doors of Coover, the commonly used terminal rooms, and your office space. In other cases, your major professor will determine what keys you will need during your program of study.
To obtain keys, please complete the Request for Keys form and return it to the front desk secretary in 2215 Coover Hall. At this time, a keycard will be issued to you. This keycard must be taken to the Key Issue office where Facilities and Planning Management (FPM) will issue the actual key(s) to you.
