Departmental Forms

Department forms were developed to be used before Graduate College degree program forms are filed, or for record keeping proposes.  

A Request to Change Degree Programs is used when changing from a M.S. degree to a Ph.D. or when "stopping off" or "stopping out" of your Ph.D. program. The Request to Declare or Change Adviser/Major Professor could be filed to declare a co-major professor or to change from one adviser to another within the department. See the Adviser/Major Professor section of Guidelines for more information on changing advisers. Both forms require signatures from your admitting adviser and your new major professor and then submitted to ECpE Student Services for approval.

Provisionally admitted students complete the Request to Change From Provisional to Full Admission form when background course deficiencies have been met. Complete instructions are listed on the form. When stating that a course was taken at another institution, the Iowa State University course supervisor must approve the request. NOTE: If admitted provisionally to take first year graduate level coursework, this form is NOT required and ECpE Student Services will automatically change your status at the end of the first year of study.

When registering for the Electrical Engineering or Computer Engineering Special Topics course, the form EE/CprE 590 Special Topics Form needs to be completed and submitted to ECpE Student Services. This form is a signed contract between you and the course instructor outlining expectations for fulfilling the course requirements. After receiving the form, the Graduate Secretary will give you the correct registration number. If the course is to be included on the Program of Study, a final written report will be need to be submitted to ECpE Student Services.

In cases of emergencies, we need to be able to contact relatives or friends that can help you. Please fill out the Emergency Sheet and return it to ECpE Student Services. Please keep us informed of changes in the information throughout your time with us.

In most cases, you will be assigned keys for the outside doors of Coover, the commonly used terminal rooms, and your office space. In other cases, your major professor will determine what keys you will need during your program of study. To obtain keys, please complete the Request for Keys form and return it to the front desk secretary in 216 Nuclear Engineering. At this time, a keycard will be issued to you. This keycard must be taken to the Key Issue office where Facilities and Planning Management (FPM) will issue the actual key(s) to you.
